Multistage treatment of an open tibia fracture
Problem description: High risk and complex trauma architecture
A patient came to us with an open fracture of both tibia bones. The complexity of such cases lies in the combination of bone crushing and significant soft tissue damage. In case of open fractures, the primary task is not only fixation, but also the fight against infectious risks. The patient required immediate intervention, which could not be performed in one step due to the condition of the tissues.
Solution: A multi-step recovery strategy
Treatment was carried out according to the principles Damage Control (damage control):
- I stage: PHO (primary surgical treatment) of the wound and installation of an external fixation device (AFF) to stabilize fragments and control tissue healing.
- II stage: After complete cleaning of the wound and removal of edema, a transition to internal osteosynthesis was performed — replacement of the device with a modern metal structure (plate or pin) for stable and final bone fusion.
This approach made it possible to minimize the risk of complications and prepare a reliable foundation for bone fusion.
